Designating a beneficiary for your life insurance and retirement savings accounts is critical! Ensure these benefits are paid to the person or persons you choose in the event of your death. Avoid legal disputes over money, by naming a benefits recipient or by updating your current beneficiary information.
Declaring your beneficiaries when you first enroll in benefits is as important as updating the info when your circumstances change (marriage, divorce, death, remarriage, etc.). Designee names may be modified at any time.
NOTE: Naming and updating beneficiaries MUST be done separately for each of the following accounts if you are enrolled:
457 Beneficiary Designation Form
Beneficiary for Final Paycheck - Civilian
Beneficiary for Final Paycheck - Fire and Police
Life Insurance Beneficiary Designation Form
TMRS Beneficiary Designation Form
