Have you ever needed medical care, but weren’t familiar with all of the features and options of your health benefits? Medical care needs are different and choosing the best place for treatment differs. Here’s what you should remember when deciding the right care whenever needed!

Your Primary Care Doctor or Provider

  • Cares for most of the basic health care needs
  • Handles wellness checkups, preventive care and immunizations
  • Cares for basic health problems like colds, flu or minor injuries
  • Works with specialists to ensure you get the care needed
    $ Copay: $25 ($5 if your primary care doctor is in the Catalyst Health Network including Village Health Partners)

Specialist Doctor

  • Provides condition-specific care; NO referral needed unless required by the specialist
  • Airrosti* offers musculoskeletal pain management 
    $$ Copay: $40 (Airrosti Copay: *$15)

Urgent Care

  • Use if unable to reach your doctor; no appointment is needed
  • Handles minor cuts, burns, broken bones, muscle sprains or strains 
  • Treats coughs, fevers, flu, skin rashes, sore throats and urinary tract infections 
    $$ Copay: $50

Emergency Room (ER)

  • Go for life-threatening health problems
  • Use for chest pain, trouble breathing or bleeding that won’t stop 
  • Treats poisoning, other major injuries or sudden numbness of the arms or face
  • Call 911 for true emergencies or go to the nearest ER right away 
    $$$ Copay: Deductible + $200

  • Bladder/urinary tract infection    
  • Pink eye
  • Bronchitis               
  • Rash
  • Cold/flu                                       
  • Sinus infection

  • Migraine                                      
  • Sore throat
  • Stomach ache
    $ Copay: $5

  • Email for health coverage or billing questions. Include your name, member ID, date of birth and supporting documents for billing questions. The Service Team responds within 24-48 hours
