It is never too early to start thinking about your health and its always nice to save some money at the same time! Earn your Connect4Health (C4H) Premium Incentive for 2023 by completing two steps between September 1, 2021 and August 31, 2022.
You will save $600 next year on your medical insurance premium rates for Employee Only and Employee plus Children coverage. You will save $1200 on your medical premium rates if both you and your spouse/domestic partner complete your incentive steps if you’re enrolled in Employee + Spouse or Family Coverage.

What Do I Need to Do?
If you are on the health plan as of 1/1/22 and your spouse is as well, you both will need to complete a preventive exam and biometric screening before the deadline of August 31, 2022.
You can choose to complete an annual exam, well- woman with an OBGYN, colonoscopy, or mammogram.
Complete your biometric screening by bringing your biometric screening form with you to your preventive exam or have LabCorp draw your blood. For more information about your biometric screening options, click here.
