Less than a month remains and approximately 600 spouses or domestic partners have yet to complete the requirements to earn 2019 health premium discounts. Each employee, spouse and domestic partner MUST have one preventive exam and one biometric screening for the premium incentive program!
If you and your spouse or domestic partner are enrolled on the City’s medical plan as of January 1, 2018, there are ONLY two preventive items required in order to earn your premium incentive for 2019:
1. Preventive Exam – complete one of the following before September 30, 2018
(Try Village Health Partners for last minute appointments.)* You can schedule your annual physical anytime during the calendar year.
2. Biometric Screening – complete in 1 of 2 ways:
NOTE: If you participated in the City’s onsite screenings, you’ve completed this requirement.
You don’t need to notify anyone when you’ve completed these two steps! Michelle Gifford, senior wellness coordinator, will send notices by email in the coming weeks about your premium incentive status. Watch for your email report!
