One ringy-dingy, two-ringy-dingy…“Hello, this is UHC calling with an important message for YOU!” It may seem unusual, but the next time your phone’s caller ID reads “UnitedHealthcare (UHC),” there’s a good chance it’s an offer to connect you with information to help manage or improve your health. These are not sales calls; rather, the calls are used to extend invitations to members identified as those who could benefit from a conversation with a nurse. Reasons for these calls can include:
A care coordination nurse may call after you return home from a hospital stay to help confirm you have after-care instructions, medication and medical equipment or in case you have other needs or questions.
UHC’s nursing staff receives referrals for patients who have certain medical conditions such as diabetes or coronary artery disease. Based on this information, you may get a call to help connect you to tools and resources to manage your health.
When you find out you are pregnant, a nurse from the Healthy Pregnancy program reaches out to offer educational services and health resources.
Another clinical outreach program UHC provides is HealtheNotes. This program helps you to improve your health and to remember the importance of preventive care; plus provides personalized care opportunities and information. One of the best things about HealtheNotes is you’ll receive them automatically in the mail when there’s a health recommendation of benefit to you. For example, if you have diabetes and medicine guidelines suggest lab tests twice a year, you may receive a HealtheNotes message reminding you to schedule tests; or, if you have a chronic kidney disorder, you may receive notifications about opportunities to better manage your condition (i.e., tips to take medications as prescribed).
HealtheNotes reminders may also focus on preventive care and screenings (i.e. reminders on women’s health: breast and cervical cancer screenings, or childhood and teen vaccines).
HealtheNotes messages are based on information UHC receives from your care providers and pharmacies. The goal is to provide you with all the information available to help ensure you receive the best care possible.
