Remember Dr. Marcus Welby M.D.? Doctors no longer make house calls like he did in the 1970s. You must schedule appointments to see your physician these days and it usually takes weeks to get on the calendar for a visit. Don’t wait much longer to set an appointment for your annual exam. The exam can be booked any time during the calendar year, which ends September 30.
If you are enrolled on the City’s medical plan as of January 1, 2018, there are ONLY two checklist items required in order to earn your premium incentive for 2019:
Download a blank biometric screening form and take it to your next preventive exam visit; OR Request a LabCorp Voucher; Access the forms or request a voucher at uswellness.com/plano
NOTE: If you participated in the City’s onsite screenings, you’ve completed this requirement.
You don’t need to notify anyone when you’ve completed these two steps!
Connect4Health receives participation reports only from UHC and US Wellness to confirm who has completed the requirements. No Personal Health Information is included in the participation reports that Human Resources receives.

As a reminder, if you have a spouse or domestic partner enrolled on the City medical plan as of January 1, 2018, they to need to complete the 2-step requirement to earn their premium incentive for 2019.
