Bring Your A-game to Your Doctor Visit

Annual physical exam

Many of you are scrambling to secure appointments with your primary doctors about now. We’re nearing the deadline to complete Connect4Health Premium Incentive requirements (September 30) and getting an annual physical or well-woman/man exam is on the TO-DO list.

The reality is it takes longer to even set an appointment these days. When you do get to see the doctor, you’ll want to make the best use of your time at the exam. Go prepared. Here’s a checklist of things (as recommended by UnitedHealthcare) to maximize this yearly wellness visit:

  • Bring your health plan I.D. card and a photo I.D.
  • Bring a list of all medications – even over-the-counter stuff; include dosage.
  • Bring a record or history of your health issues: surgeries, diseases and past illnesses.
  • Bring a list of questions for the doctor. This is your opportunity for face-to-face answers and people often remember questions AFTER they leave the appointment.
  • Bring a relative or friend. Some doctor visits may require some backup to take notes or drive you home.
  • Bring a blank biometric screening form to be filled out if you haven’t already taken care of it. This is also a requirement of the C4H health premium incentive program.

Be sure to speak with your physician about symptoms you may be experiencing that don’t necessarily show up on tests. It’s your health – take control.
