Schedule Your Onsite Biometric Screening – Save Time and Money

The City is committed to the health and wellness of every employee and is again offering onsite biometric screenings. Do you know your numbers? Make your appointment today for March or April before all of the good slots fill.

Go to to select an onsite biometric screening appointment at one of several convenient locations!

The Connect4Health (C4H) biometric onsite screenings cover:

  • Total cholesterol, HDL, LDL, triglyceridesBiometric screening data
  • Glucose and blood pressure
  • BMI, waist circumference and body composition
  • A1c for qualified participants

When you participate in one of our onsite events, you’ll receive your results immediately and be able to discuss them with a trained healthcare professional.

If you are enrolled on the City’s health plan as of January 1, 2020 and would like to save some money on your premium rates for 2021, completing your biometric screening satisfies one of only two 2021 Connect4Health (C4H) Premium Incentive requirements!

Spouses or domestic partners enrolled on the City’s health plan as of January 1, 2020 can participate in onsite screening as well. This helps to save on their 2021 premium rates too!

Now is the time to get ahead of the September 30, 2020 deadline and schedule a screening that fits your busy life. If you aren’t able to attend an onsite screening, go to to see other options:

  • Request a lab voucher for offsite screenings at LabCorp
  • Print a Biometric Screening Form for the doctor to complete at your annual exam
